Position Description
Veridia Health is seeking a full-time Patient Care Coordinator to help deliver a seamless, patient-centered healthcare experience. This role serves as the primary liaison between patients, providers, and the healthcare system by coordinating care, managing patient communications, maintaining medical records, and supporting day-to-day clinical operations. The ideal candidate is organized, compassionate, and committed to providing exceptional service within a personalized, preventive care model.
Responsibilities include:
- Attending patient appointments and documenting care plans
- Coordinating appointments, preventive screenings, specialist referrals, and diagnostic testing
- Maintaining accurate electronic medical records (EMR) and clinical documentation
- Managing medication and supplement records
- Communicating with internal teams and outside healthcare providers
- Assisting with insurance authorizations, referrals, and appeals
- Onboarding new patients and serving as their primary point of contact
- Responding to patient questions and supporting care plan follow-up
- Tracking patient tasks and follow-ups using project management tools
- Identifying workflow improvements to enhance operational efficiency
- Collaborating with the clinical team to support patient engagement and coordinated care
Position Requirements
- Bachelor’s degree required
- Strong interest in healthcare and patient-centered medicine
- Excellent organizational skills and attention to detail
- Outstanding written and verbal communication skills
- Ability to build strong patient relationships while maintaining confidentiality
- Ability to thrive in a fast-paced, collaborative environment
- Experience with EMR systems and healthcare technology preferred
- Familiarity with U.S. healthcare operations and insurance processes preferred
- Strong critical thinking and problem-solving abilities
Time Commitment

Compensation
$55,000–$80,000 per year
